Maternity Support Services and Infant Case Management are preventive health and education services to support healthy pregnancies and healthy babies. Specially trained Community Health Nurses visit families in their home throughout the pregnancy and continue to visit until the child is 2 years old. The nurses visit weekly and bi-weekly during the pregnancy, infancy and toddler period. The education curriculum includes information during each phase of the development. This relationship based model builds on the parent’s strengths supporting the parent child relationship, healthy parent-child outcomes and goals for the future helping first time parents succeed. This program is free to eligible women.

Serves those who are pregnant or have children under 1 yr old that receive medical insurance through the State of Washington. Provides professional in-home visits by a staff of licensed nurses, counselors, and dietitians for low income or at-risk pregnant women who are facing barriers to having a healthy pregnancy. Works with women during pregnancy and, depending on her needs, into her baby's first year of life. Each client is provided with one-on-one counseling and education, in her home or place of residence, to help her have a healthy pregnancy and take needed steps to prepare and provide for her family. Also assists with resources life skills classes, community events, vital needs (such as diapers, car seats/cribs and food), job training, and many other services and resources to help women succeed. Moms can also continue to attend events and gatherings, even after they are not currently in the program. May also provide information on Safe Sleep and access to cribs to those receiving services.

Program designed to help pregnant women receive the medical and social services they may need. All pregnant women covered under Medicaid (WA Apple Health) are eligible for services. Offers a team that helps advise and support mothers through their pregnancy. Team includes a: - Registered Nurse - Nutritionist - Behavioral Health Specialist - Community Health Worker - an Infant Case Manager. Services are provided wherever convenient for participants whether that be in home, clinic or in the community. Services may include, but are not limited to: - free pregnancy testing - assistance in obtaining early and regular medical care - assistance in applying for Medicaid Medical Insurance/Apple Health - nursing information and education about pregnancy, labor and delivery - diet and nutrition counseling - information and education about baby care and parenting skills - assistance in accessing dental and vision care - childbirth education classes - care seat safety and car seats - family planning services - assistance in obtaining paternity information - assistance with housing - transportation to all medical and dental appointments - substance abuse counseling - lactation, breastfeeding support and breast pumps - free crib program - perinatal, prenatal and postpartum anxiety/depression support and counseling.

Maternity Care - Provides routine and high-risk pregnancy care - Includes prenatal care and delivery at a local hospital. Maternity Support Services - MSS/Infant Case Management - ICM Households using Apple Health may also qualify for: - Referrals to community programs and services - Pregnancy and infant care education - Assistance with finding housing, employment, clothing, furniture, transportation - Assistance with finding financial assistance. - for those eligible, support services continue until the baby's first birthday. May also provide information on Safe Sleep and access to cribs to those receiving ICM services.

Offers free nurse home visits for caregivers and their newborns. Provides a medical check-up on the birth parent and the baby, as well as education for the family, lactation assistance, answers to any questions the family may have, and connection to local resources that help families thrive. Also offers weekly support with a virtual and in-person Baby Lounge - a place for all families with babies to connect and learn together as parents. Groups are facilitated by a health professional. There are child care stipends available, and refreshments are provided.

Black Infant Health Works with African American churches and Health Ministers to support pregnant and parenting mothers and their families. Provides health education and linkages with medical and community resources. Children with Special Health Care Needs - CSHCN Connects families of children with special health care needs to providers and services. Provides case management, care coordination and referral to community-based programs. Maternal Child Outreach Team - MCOT Works to reduce high rates of infant mortality in Pierce County. Connects newly pregnant mothers to health insurance including Medicaid, smoking cessation and other support services. Maternity Support Services - MSS Promotes healthy pregnancies and babies through nurse home visiting. Provides preventative health and education services to women who are enrolled in Medicaid. Nurse Family Partnership - NFP Provides nurse home visiting services for first-time pregnant low-income women. Improves birth outcomes, child health and development, and economic self-sufficiency of families. Referrals must be received by 26th week of pregnancy.

Provides home visits to first time parenting moms and their families throughout pregnancy and until the child is two years of age. Nurses work to support families in several different domains, for example nurses visit families at home and teach about the mother’s personal health and environmental health; they guide new mothers in developing healthy and successful parenting skills while promoting social, emotional, and physical development; they mentor clients in establishing and meeting their school and career goals; they support clients in building a healthy support network; and they make referrals as needed to community agencies.
